Screening Colonoscopy

To Schedule - Call our office at 937.350.6700
- To schedule an appointment to see the doctor if you answered “YES” to
any of the questions at the beginning of this process.
- Or to schedule your screening colonoscopy if you answered “NO” to
all the questions in STEP 1.
- Be sure to have the following information ready when you call to
schedule your colonoscopy over the phone
- Insurance card
- A list of all medications
- A list of past
surgeries and any health conditions
- If you would like to fill out your Health History online, please use our
Athena
Health portal.
- Be sure to choose the option below "Are you new to the patient portal?" and click on CREATE AN ACCOUNT unless you have been given a PIN from our office.
